In the previous episode, Emma and Hook walked down the aisle and exchanged the rings. The couple got married. Unfortunately, even before they said "I Do," Fiona cursed Emma and Storybrooke, separating her from her loved ones after she gets married. This curse is no different from her parents' curse, who were separated shortly after their marriage.
In "Once Upon a Time" (OUAT) Season 6 finale, Henry helps his mother to break the curse before the final battle begins. According to the description of Episode 21 & 22, the heroes are gone and Emma struggles for life.
Henry wakes up to find out that Storybrooke is cursed and his mother is in mental institution fight for her life. Meanwhile, Black Fairy is the new mayor of the town and Regina is gone. Henry takes matters into his own hands and tries to help his mother revive her memory and fight Fiona.
In the meantime, Gold is clueless about Belle and tries to find out what happened to her. As for Snow White, Prince Charming, Regina, Zelena and Captain Hook, they are all trapped in a devastated fairytale land. Before everything falls to pieces, they must find their way out.
